I think the equipment, etc is fine. The problem is the donor themselves. With a heart recipient hovering over the grave with a 12% ejection fraction - they may not care.. But as one readers points out, what a disaster it would be if one was injured suddenly in a car wreck or a fall and required emergency surgery and transfusion and then gets AIDS or COVID issues or Hep C.. Realize that you will have had to sign for blood (or your relative). I have operated on Jehovah's witnesses doing heart surgery on them with no blood transfusions. Some sects of that religion- will allow the use of cardiopulmonary bypass for heart surgery, but once the pump stops, one may not restart it again and no blood may be removed and processed out of the pump like I have described. Don't ask me to explain, cause I don't understand either. But - I have followed the family request for the patient.
gonna be very hard to start up a "pureblood" donor center and blood bank. Likely would be sued, likely would be made illegal by many of the WOKE states.
I watched a video which showed the poorest, drug addicted people in USA going to sell their blood. Yes, they were questioned about their lifestyles but they just lied as they were desperate. The blood was needed for plasma which was exported to Switzerland and being given to private cancer patients being charged tens of thousands of $$$ each time. The investigative journalist followed the trail and questioned everyone involved regarding where they thought the blood came. As usual, most assumed it was from healthy volunteers. Some knew. This was filmed prior to the plandemic.
just go to one of these blood donor collection sites. Park your car and watch who is coming in to donate. Pretty obvious we have a problem. And have for decades. That is why when folks were Jehovah's Witnesses and refused blood, I would still operate but made sure they understood and signed of on the increased risk if their hemoglobin got too low and they refused blood transfusion. Actually I don't ever remember a patient dying because of that decision. So the tentacles of deception from the drug cartels and the illegals bringing drugs across from the south - have serious impacts most people never think about.
I'm from UK and we don't pay anyone, all voluntary donations. However, if people were paid, it would be the same. Watching from car park, the journalist interviewed people leaving. He was refused interviews with staff at these centres. I've also read of successful ops on JWs without transfusion. The drugs problem is huge.

I am curious whether autotransfusion donations could be contaminated by e.g. tubes with residue but I haven't been up close to inspect these machines.
All recipients of all organs or blood should be fully informed as to the status of that donation. Someone needing a heart transplant probably is going to be ok with Hep C since there are now effective treatments and the alternative is worse, but they should still get to make that choice on their own. However the HIV thing is NOT ok, and if I am in a car accident and need blood, I am NOT ok with not knowing the HIV/Hep C status of that blood. Save me from a car accident just to find out I've been given a truly debilitating and expensive, lifelong illness and possibly a death sentence anyway? This is not a fair or ethical choice! There's gotta be some accountability.

Just like how we secured international air travel at the beginning of Covid by screening all passengers with the questionnaire, "Have you been coughing?" No? Okay – board the plane.
